Issuing Proof of Vaccination certificates

Those who have been vaccinated for COVID-19 can get proof of vaccination. As it displays your vaccination history, please store it securely.

For health care providers who got a vaccine with a “vaccine-voucher medical pre-exam” form, the attached ‘Record for Vaccination for COVID-19’ is your proof of vaccination.

Reissuing the certificate of vaccination

People in the following categories may be eligible to apply for a re-issuance of proof of vaccination.

Eligibility

Those whose proof of vaccination has been damaged, destroyed or is missing due to whatever circumstance.

Required documents

1. Application for issuance of COVID-19 vaccine voucher
2. Identity verification (driver’s license, health insurance card etc)
3. Commission form (only in cases where applying on behalf of someone else who is not living in your household)

How to request

By mail

Send all required documents to the address below.

At the service window

Bring the required documents with you and come to the service counter below. Service hours: 8:30 – 17:15

Furthermore, only applicants applying for themselves, household members or in possession of a Commission Form can apply at the service window.

Delivery method

The form will be issued after confirmation of your vaccination status.

We will mail it within 5 days of accepting your documentation. However, it’s possible that in cases where it’s difficult to verify your vaccination status, it may take longer.

For those who need a “Vaccine passport” in order to travel overseas

We will also issue official proof of vaccination as required by some countries and regions to determine how much to mitigate disease-prevention measures.

※ This Proof of Vaccination form is for those who are traveling overseas.

If you are not traveling overseas, please use the right-hand side of your Vaccine Voucher or‘Record for Vaccination for COVID-19’ that you got at the time of vaccination. For more information, see the “Proof of Vaccination certificates” section above.

Eligibility

Those for whom 1～3 apply.

1. Those who received a COVID-19 vaccine according to the vaccine laws (regular local vaccination with a vaccine voucher, or advanced vaccination with a “vaccine-voucher medical pre-exam”)
2. Those requesting this document because it will allow relief from some restrictions at an overseas destination they’re planning to travel to
3. Current or former residents of Shikokuchuo city who received a vaccine voucher in this city (whether or not you got your vaccination here)

Steps to apply

1. Application by mail

   Mail the required documents to this address:

2. Application at the service window:

   Bring the required documents to the service window between 8:30 and 17:15.

   Furthermore, only applicants applying for themselves, household members or in possession of a Commission Form can apply at the service window.

Application at the service window

1. Application for delivery of a Proof of COVID-19 Vaccination
2. Passport
3. Vaccine Voucher
   ※Keep the rest of your coupon without discarding it.
   （Here’s a sample of the Vaccine Voucher in pdf form）.
4. Record of Vaccination (for medical providers who got vaccinated after a ‘vaccine pre-exam’ instead of with a Vaccine Voucher.)

   【For those requesting delivery of the Proof of COVID-19 Vaccination by mail】

5. Documents for identity verification (ID with your return address printed on it: a Japanese driver’s license, Residence Card, My Number card etc.)
6. Return envelope (with your name and address written on it)

   【For those who don’t have the Vaccine Voucher form】

7. Document that can verify your “My Number” (i.e. Individual Number card, Individual Number notice, Resident Certificate etc)

   【When a third party is applying (for someone else)】

8. Authorization form

   【When your passport information differs from your current situation (name, gender etc.)】

9. Documentation of your previous name or gender (copy of your My Number card, Driver’s License, family registry, Resident Certificate etc)

    

Delivery method if you apply at the service window

After checking the required documents, if nothing is lacking we will issue the document within five work days. However, if inquiries into the vaccination records take an unusually long time, we may not be able to issue the document within five work days, so please apply well in advance of when you need it.
